Accountability processes

Hi everyone,

Our community is working on developing an explicit accountability process. We are hoping to have a clear process for lots of situations, but have a particular interest in processes for asking someone to leave the community. After some brainstorming we realized we would benefit from seeing some examples. Does anyone have an example from your community that you can share with us?
